In the fourth section of each deed, or, if it is not so divided, at the foot thereof, shall be stated:
the name and the initials of the registrar of births, deaths, marriages and registered partnerships (ambtenaar van de burgerlijke stand) who has drawn up the deed;
the place and the day on which the deed was drawn up;
the signatures of the Registrar (ambtenaar van de burgerlijke stand) and, insofar as applicable, of the parties to the deed, as well as of the witnesses, or the statement referred to in Article 20, paragraph 2, of this Decree.
In the fourth part of each deed of registration, the data referred to in paragraph 1, under a and b, shall be stated, as well as the signature of the registrar of births, deaths, marriages and civil partnerships.
In het vierde gedeelte van elke akte, dan wel, indien deze niet is ingedeeld, aan de voet ervan worden vermeld:
de naam en de voorletters van de ambtenaar van de burgerlijke stand die de akte heeft opgemaakt;
de plaats en de dag waarop de akte is opgemaakt;
de handtekeningen van de ambtenaar van de burgerlijke stand en, voor zover van toepassing, van de partijen bij de akte, alsmede van de getuigen, dan wel de verklaring, bedoeld in artikel 20, tweede lid, van dit besluit.
In het vierde gedeelte van elke akte van inschrijving worden de in het eerste lid, onder a en b genoemde gegevens vermeld, alsmede de handtekening van de ambtenaar van de burgerlijke stand.
