in forceThe obligation to conclude a health insurance contract · § Commencement and termination of health insurance
The health insurer shall provide the policyholder and, if this is a person other than the policyholder, the insured, with a health insurance policy as soon as possible after the conclusion of the health insurance agreement and subsequently prior to each calendar year.
If the health insurance terminates, the health insurer shall provide the policyholder and, if this is a person other than the policyholder, the insured person with proof of the termination of the health insurance, on which shall be recorded:
name, address, place of residence and citizen service number (burgerservicenummer) of the insured;
name, address and place of residence of the policyholder;
name, address and place of residence of the health insurer;
the day on which the health insurance ends;
or whether a voluntary deductible applied to the insured on that day and, if so, with what effective date, for what amount, and with what discount granted in connection therewith.
If the health insurance terminates for the reason referred to in Article 6, paragraph 1, point (d), this shall be noted on the proof referred to in the second paragraph.
De zorgverzekeraar verstrekt de verzekeringnemer en, indien deze een ander is dan de verzekeringnemer, de verzekerde zo spoedig mogelijk na het sluiten van de zorgverzekering en vervolgens voorafgaande aan ieder kalenderjaar een zorgpolis.
Indien de zorgverzekering eindigt, verstrekt de zorgverzekeraar de verzekeringnemer en, indien deze een ander is dan de verzekeringnemer, de verzekerde een bewijs van het einde van de zorgverzekering, waarop worden aangetekend:
naam, adres, woonplaats en burgerservicenummer van de verzekerde;
naam, adres en woonplaats van de verzekeringnemer;
naam, adres en woonplaats van de zorgverzekeraar;
de dag waarop de zorgverzekering eindigt;
of voor de verzekerde op die dag een vrijwillig eigen risico gold en zo ja, met welke ingangsdatum, voor welk bedrag en met welke in verband daarmee verleende korting.
Indien de zorgverzekering eindigt om de in artikel 6, eerste lid, onderdeel d, genoemde reden, wordt dat op het in het tweede lid bedoelde bewijs aangetekend.
