The deed of choice of name, which is drawn up on the basis of a declaration by the child, shall state:
the surname, the given names, the place and the date of birth of the parents;
the surname of the child prior to the making of the choice of name, the first names, the place and the date of birth of the child;
the surname chosen by the child.
The marginal note of the deed of choice of name shall state the surname chosen by the child.
De akte van naamskeuze die op grond van een verklaring van het kind wordt opgemaakt, vermeldt:
de geslachtsnaam, de voornamen, de plaats en de dag van geboorte van de ouders;
de geslachtsnaam van het kind vóór het doen van naamskeuze, de voornamen, de plaats en de dag van geboorte van het kind;
de door het kind gekozen geslachtsnaam.
De latere vermelding van de akte van naamskeuze vermeldt de door het kind gekozen geslachtsnaam.
