The birth certificate shall state in the first part consecutively:
the surname or provisional surname of the child;
the first names or provisional first names of the child;
the day and, in so far as known, the hour and the minute of the birth;
the place of birth;
the sex of the child;
The deed shall state in the second part consecutively:
the surname and the given names of the father or the mother from whom the child was not born;
the surname and the given names of the mother from whom the child was born. This person may, if they were of the male sex at the time of the child's birth, be designated in the deed as the parent from whom the child was born, in accordance with the deed of acknowledgement or at their petition;
The deed shall state in the third section consecutively:
if applicable, that the surname has been chosen;
insofar as known, the place and the date of birth of the father or the mother from whom the child was not born and of the mother or, in accordance with the preceding paragraph, under (b), the parent from whom the child was born;
the surname and the given names, as well as the place and the date of birth of the declarant;
if applicable, that the first names have been given ex officio by the registrar of births, deaths, marriages and registered partnerships.
De akte van geboorte vermeldt in het eerste gedeelte achtereenvolgens:
de geslachtsnaam dan wel voorlopige geslachtsnaam van het kind;
de voornamen dan wel voorlopige voornamen van het kind;
de dag en, voor zover bekend, het uur en de minuut van de geboorte;
de plaats van geboorte;
het geslacht van het kind;
De akte vermeldt in het tweede gedeelte achtereenvolgens:
de geslachtsnaam en de voornamen van de vader of de moeder uit wie het kind niet is geboren;
de geslachtsnaam en de voornamen van de moeder uit wie het kind is geboren. Deze persoon kan, indien hij ten tijde van de geboorte van het kind van het mannelijk geslacht was, in de akte overeenkomstig de akte van erkenning of op zijn verzoek worden aangeduid als ouder uit wie het kind is geboren;
De akte vermeldt in het derde gedeelte achtereenvolgens:
indien toepasselijk, dat de geslachtsnaam gekozen is;
voor zover bekend, de plaats en de dag van de geboorte van de vader of de moeder uit wie het kind niet is geboren en van de moeder of, overeenkomstig het voorgaande lid, onder b, de ouder uit wie het kind is geboren;
de geslachtsnaam en de voornamen alsmede de plaats en de dag van geboorte van de aangever;
indien toepasselijk, dat de voornamen ambtshalve door de ambtenaar van de burgerlijke stand zijn gegeven.
