For a juvenile in a state institution, an educational pathway plan shall be drawn up in which the educational objectives and results or interim results are recorded.
The educational trajectory map provides an up-to-date overview of the educational history, test and assessment data, the educational goals that are being planned, and the goals or intermediate goals that have already been achieved.
The educational pathway map consists of:
the intake, including the conclusions and recommendations,
the educational history of the minor,
the educational trajectory followed by the minor,
the progress of the educational trajectory,
the continuation of education following the stay in the institution.
Voor de jeugdige in een rijksinrichting wordt een onderwijstrajectkaart opgesteld waarin de onderwijskundige doelstellingen en resultaten of tussenresultaten zijn vastgelegd.
De onderwijstrajectkaart geeft een actueel beeld van het onderwijsverleden, test- en toetsgegevens, de onderwijsdoelen die worden gepland, en de doelen of tussendoelen die reeds zijn bereikt.
De onderwijstrajectkaart bestaat uit:
de intake, waaronder de conclusies en aanbevelingen,
de onderwijshistorie van de jeugdige,
het onderwijstraject dat door de jeugdige wordt gevolgd,
de voortgang van het onderwijstraject,
de voortgang van het onderwijs na het verblijf in de inrichting.
