The declaration regarding the right of residence of the prospective spouse (echtgenoot) or registered partner (geregistreerd partnerschap) as referred to in Article 44, paragraph 1, under i, of Book 1 of the Civil Code (Burgerlijk Wetboek), shall state, insofar as applicable, the following data concerning the person to whom the declaration relates:
his nationality;
a description of his identity document, stating the number, the place and the date of issue;
a description of the documentary evidence of permanent residence abroad;
the following data concerning the children to whom he stands in a family law relationship: surname, forenames, date, place and country of birth, nationality, place of residence or stay, as well as the surname and forenames of the other parent who stands in a family law relationship to the children;
the following data regarding his previous spouse or spouses or previous registered partner or registered partners, his spouse or registered partner: surname, first names, date, place and country of birth, nationality, place, country and date of the solemnisation of the marriage or of the marriages or of registration of the partnership or of the partnerships, place, country, date and ground for the dissolution of the marriage (ontbinding van het huwelijk) or of the previous marriages and for the termination of the previous partnership or of the previous partnerships;
the data concerning the duration of his residence in the Netherlands;
other relevant data;
the signed statement that the data under 1° to 7° inclusive have been provided truthfully, stating the place and date;
The registrar of births, deaths, marriages and registered partnerships shall retain the declaration referred to in the first paragraph for a period of twelve years for the purpose of verification by the Immigration and Naturalisation Service.
De verklaring omtrent het verblijfsrecht van de aanstaande echtgenoot of geregistreerde partner als bedoeld in artikel 44, eerste lid, onder i, van Boek 1 van het Burgerlijk Wetboek, vermeldt voor zover van toepassing, de volgende gegevens betreffende de persoon op wie de verklaring betrekking heeft:
zijn nationaliteit;
een omschrijving van zijn identiteitsbewijs, met vermelding van het nummer, de plaats en de datum van afgifte;
een omschrijving van het bewijsstuk van permanent verblijf in het buitenland;
de volgende gegevens over de kinderen tot wie hij in familierechtelijke betrekking staat: geslachtsnaam, voornamen, datum, plaats en land van geboorte, nationaliteit, woon- of verblijfplaats, alsmede de geslachtsnaam en voornamen van de andere ouder, die tot de kinderen in familierechtelijke betrekking staat;
de volgende gegevens over zijn eerdere echtgenoot of echtgenoten of eerdere geregistreerde partner of geregistreerde partners, zijn echtgenoot of geregistreerde partner: geslachtsnaam, voornamen, datum, plaats en land van geboorte, nationaliteit, plaats, land en datum van de voltrekking van het huwelijk of van de huwelijken of van registratie van het partnerschap of van de partnerschappen, plaats, land, datum en grond van de ontbinding van het eerdere huwelijk of van de eerdere huwelijken en van de beëindiging van het eerdere partnerschap of van de eerdere partnerschappen;
de gegevens over de duur van zijn verblijf in Nederland;
andere relevante gegevens;
de ondertekende verklaring dat de gegevens onder 1° tot en met 7° naar waarheid zijn verstrekt, met vermelding van plaats en datum;
De ambtenaar van de burgerlijke stand bewaart de in het eerste lid bedoelde verklaring gedurende twaalf jaar ten behoeve van verificatie door de Immigratie- en Naturalisatiedienst.
