A petition for a change of the surname or for the determination of the surname or forenames of a person shall be submitted by him or his legal representative by means of a petition addressed to Us.
2.
The petition may be submitted to Our Minister of Justice.

Een verzoek tot wijziging van de geslachtsnaam of tot vaststelling van de geslachtsnaam of voornamen van een persoon wordt door hem of zijn wettelijke vertegenwoordiger gedaan bij een tot Ons gericht verzoekschrift.
2.
Het verzoekschrift kan worden ingediend bij Onze Minister van Justitie.
