In the heading of each act, the word "Netherlands", followed by the letters (NL), the type of act and the year in which the act was drawn up, shall be stated. In the heading of an act of birth, of marriage (huwelijk), of registration of a partnership, of conversion of a registration of a partnership, of acknowledgement, of denial of paternity or maternity by the mother from whom the child was born, of choice of name and of death, a number shall also be stated, in accordance with regulations to be issued by the Minister of Justice.
In het hoofd van elke akte worden het woord "Nederland", gevolgd door de letters (NL), de aktesoort en het jaar waarin de akte is opgemaakt, vermeld. In het hoofd van een akte van geboorte, van huwelijk, van registratie van een partnerschap, van omzetting van een registratie van een partnerschap, van erkenning, van ontkenning van het vaderschap of moederschap door de moeder uit wie het kind is geboren, van naamskeuze en van overlijden wordt tevens een nummer vermeld, overeenkomstig door de Minister van Justitie te geven voorschriften.
