The grounds for the objection or appeal, as referred to in Article 6:5, paragraph 1, subparagraph d, of the General Administrative Law Act (Algemene wet bestuursrecht), shall be stated in a separate annex insofar as they relate to medical data.
De gronden van het bezwaar of beroep, bedoeld in artikel 6:5, eerste lid, onderdeel d, van de Algemene wet bestuursrecht, worden in een aparte bijlage vermeld voorzover ze betrekking hebben op medische gegevens.
