A declaration of renunciation of Netherlands nationality as referred to in Article 15, paragraph 1, opening words and under (b), or as referred to in Article 16, paragraph 1, opening words and under (b), of the Kingdom Act shall be made in writing. The declaration shall state that the person making the declaration is aware of Article 16, paragraph 1, opening words and under (d), and paragraph 2 of the Kingdom Act.
To the declaration of renunciation shall be added, if necessary, proof that the person making the declaration and, where applicable, the minors who share in his renunciation possess another nationality.
If the declaration concerns a minor, proof of the nationality of the father and mother or adoptive parent shall, if necessary, also be attached.
Een verklaring van afstand van het Nederlanderschap als bedoeld in artikel 15, eerste lid, aanhef en onder b, of als bedoeld in artikel 16, eerste lid, aanhef en onder b, van de Rijkswet wordt schriftelijk afgelegd. De verklaring vermeldt dat de persoon die de verklaring aflegt, bekend is met artikel 16, eerste lid, aanhef en onder d, en tweede lid van de Rijkswet.
Aan de verklaring van afstand wordt zo nodig een bewijs toegevoegd dat de verklarende persoon en, in voorkomend geval, de minderjarige personen die in zijn afstand delen een andere nationaliteit bezitten.
Indien de verklaring een minderjarige betreft, wordt zo nodig tevens een bewijs van de nationaliteit van de vader en moeder of adoptiefouder toegevoegd.
