in forceThe physical characteristics referred to in Article 2, paragraph 1, under (b), of the Act, as known at the time of the provision of sperm or egg cells, concern:
stature;
weight;
skin colour;
colour of the eyes;
hair colour and hair type.
The training as referred to in Article 2, paragraph 1, under (b), of the Act concerns:
educational programmes or studies that have been completed or are still being pursued by the donor at the time of the provision of sperm or egg cells. The profession as referred to in the aforementioned part of the article concerns the profession exercised by the donor at the time of the provision of sperm or egg cells.
The data concerning the social background and a number of personal characteristics, as referred to in Article 2, paragraph 1, under (b), of the Act, as known at the time of the provision of sperm or egg cells, shall concern:
age;
civil status
family composition;
a description, drawn up by the donor himself, of his characteristic qualities and personality traits.
De fysieke kenmerken, bedoeld in artikel 2, eerste lid, onder b, van de wet, een en ander zoals bekend op het tijdstip van de terbeschikkingstelling van zaad- of eicellen, betreffen:
lichaamslengte;
gewicht;
huidskleur;
kleur van de ogen;
haarkleur en type haar.
De opleiding als bedoeld in artikel 2, eerste lid, onder b, van de wet betreft:
opleidingen of studies die op het tijdstip van de terbeschikkingstelling van zaad- of eicellen door de donor zijn voltooid dan wel nog worden gevolgd. Het beroep als in genoemd artikelonderdeel bedoeld, betreft het beroep dat de donor op het tijdstip van de terbeschikkingstelling van zaad- of eicellen uitoefent.
De gegevens omtrent de sociale achtergrond en een aantal persoonlijke kenmerken, bedoeld in artikel 2, eerste lid, onder b, van de wet, een en ander zoals bekend op het tijdstip van de terbeschikkingstelling van zaad- of eicellen, betreffen:
leeftijd;
burgerlijke staat;
gezinssamenstelling;
een door de donor zelf opgemaakte beschrijving van hem kenmerkende eigenschappen en karaktertrekken.
